As a Health Access Assistant youll promote independence selfadvocacy and decisionmaking in medical and health related activities for people with I/DD. The Health Access Assistant will transport people to medical appointments and support them in taking control of their healthcare. Health Access Assistants are deeply committed to supporting people and their wellness goals while building a trusting relationship that promotes independence.
The ideal candidate for this position will be able to provide a personcentered level of care to align with the persons PersonCentered Plan. The Health Access Assistant will assist in coordinating appropriate medical related appointments providing medical services and assisting with personal care and emotional support. They will facilitate building selfesteem and confidence through positive interactions and communication with the person supported and their family. The Health Access Assistant must have a high school diploma or equivalent a valid drivers license and experience facilitating medical appointments and supporting people with disabilities.
