1.Must have a Bachelor of Science degree in Pharmacy or Doctorate of Pharmacy from an accredited college of pharmacy. Must be licensed to practice in the Commonwealth of Massachusetts.
a.Post graduate Residency training is preferred.
b.Board Certification In Pharmacotherapy Diabetes Anticoagulation or Ambulatory Care is preferred.
2.Must complete 15 continuing education credits per year to maintain license and 5 additional credits in the area of collaborative drug therapy management practice.
3.Must have 1 to 2 years experience at the Staff Pharmacist I level (see Staff Pharmacist I) or equivalent years of professional practice experience will also be considered.
4.Upon department orientation and annually thereafter must complete:
i.Safety training:
a. OSHA/Fire safety
b. Infection control
c. Safe handling of chemotherapeutic agents
ii.Competency assessments in areas of:
a. Agespecific Pharmacotherapy
b. Processspecific skills
c. Clinical skills
d. Populationspecific Pharmacotherapy
e. Aseptic technique
5.Must have a Massachusetts Controlled Substance Registration license National Provider Index number and a Drug Enforcement Administration Registration number if applicable.
6.Must possess a Doctor of Pharmacy (Pharm D) degree from an accredited college of pharmacy.
7.Must be licensed to practice pharmacy in the Commonwealth of Massachusetts.
8.Must be licensed to prescribe or qualify for licensure to prescribe in the Commonwealth of Massachusetts
9.Completion of a PGY 1 Postgraduate residency is required.
10.Board Certification in Pharmacotherapy is highly desirable
11.Certification for vaccine administration is desirable.
12.Experience practicing in an ambulatory care setting a plus but not required
13.Certification as a Diabetic Educator a plus. Willingness to obtain other clinical certifications related to pharmacotherapy management in the future is required.

Must have good oral and interpersonal communication skills to communicate effectively with members of other healthcare disciplines and other Pharmacy staff.

10.The individual in this position must demonstrate appropriate knowledge of principles of growth and development over the life span of the neonate adolescent adult and geriatric patient. In addition they must possess the ability to assess patient data relative to age specific needs and provide care as described in the departments policies/procedures.
11.Must have comprehensive knowledge of Pharmacotherapy.
12.Ability to work collaboratively as a member of the patient care team.
13.Must report performance metrics related to collaborative drug therapy management protocols to the Pharmacy and Therapeutics committee.

GENERAL SUMMARY/ OVERVIEW STATEMENT:
We are looking for a pharmacist to be a member of our team to help improve medication management population health by aggressive protocol driven management of medications for chronic diseases. The pharmacist will lead practice level activities around medications including medication reconciliation medication renewal and maximizing medication adherence.
Responsible for monitoring medication use and renewals of medications to patients in accordance with the laws and regulations governing credentialed pharmacists and the practice of pharmacy by the appropriate state and federal agencies.
Responsible for the optimization of drug therapy by identification resolution and prevention of drug related problems and improving therapeutic outcomes.
Work relatively independently as a member of the patient care team without direct supervision and within the limits of established pharmacy policies and procedures clinic policy and procedures and the regulations of the Board of Registration Pharmacy. Directs and coordinates the activities of clinical pharmacy services.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Directs and coordinates the delivery of all pharmacy services on the assigned shift without direct supervision.
2.Monitor and prescribe medications to patients in accordance with the laws and regulations of the Board of Registration in Pharmacy Board of Registration in Medicine Department of Public Health Food and Drug Administration and other regulatory agencies.
3.Responsible for the interpretation and implementation of all departmental policies and procedures.
4.Communicates with physicians and nurses on medication order discrepancies possible drug interactions or stability problems. Consults clinical staff on drug information and clinical data necessary for monitoring the patients pharmacotherapy.
5.Provides prospective evaluation and monitoring of patient drug therapy. Recommend and prescribe (in accordance with approved protocols) appropriate medication doses and adjustments as indicated by patient conditions. Identifies documents resolves and works to prevent drug therapy related problems.
6. Actively participates in the development and performance of drug use evaluations and other drug therapy monitoring initiatives.
7.Monitors drug therapy to prevent contraindications duplicate therapy drug interactions and allergic reactions. Reports and documents all adverse drug reactions to pharmacy leadership and appropriate hospital committees.
8.Documents and reports all clinical interventions adverse drug events and drug therapy profile reviews.
9.Provide information and consultation to physicians nurses and other healthcare professionals regarding appropriate drug therapy. Give advice on drug interactions side effects incompatibilities allergies and drug administration.
10.Supervise and verify the accuracy of all work completed by supportive personnel related to medication preparation storage administration and inventory.
11.Attend and participate in daily medical and multidisciplinary huddles.
12. Periodically attend and participate in journal club review inservices and case studies for afternoon pharmacist meetings.
13.Assist and contribute to the orientation and training of new personnel as assigned.
14.Prescribe (in accordance with protocols) and dispense ambulatory prescriptions as needed.
15.Provide postdischarge medication and drug history counseling to patients when appropriate.
16.Attend participate in and conduct continuing education programs and conferences to keep current with developments in the profession and to maintain license.
17.Maintain pharmaceutical care expertise through membership and participation in professional organizations and continuing education programs.
18.Maintain expertise in the departmental policies and procedures.
19.Serves as a preceptor for Pharmacy residents with the Massachusetts Board of Registration in Pharmacy.
20.Maintains skill and competency level to comply with departmental and hospital Quality Assurance standards.
21. Participates and plays leadership role in department quality improvement efforts. Plays a leadership role in Lean projects in the clinic. .
22.Participates in the training of medical nursing and allied health professionals.
23.Performs all other responsibilities as assigned.
PRINCIPAL DUTIES AND RESPONSIBILITIES SPECIFIC TO CDTM:
1.The pharmacist shall hold a current unrestricted license in good standing to practice pharmacy in the Commonwealth and currently be engaged in pharmacy practice in the Commonwealth.
2.Must have Massachusetts Controlled Substance Registration license National Provider Index number and a Drug Enforcement Administration Registration number if applicable.
3.The pharmacist shall wear a nametag which identifies him/her as a pharmacist.
4.The pharmacist shall have earned a doctor of pharmacy degree or have completed 5 years of experience as a licensed pharmacist.
5.The pharmacist shall devote a portion of practice to the defined drug therapy area that the pharmacist shall comanage.
6.The pharmacist shall complete in each year of the term of the protocol at least 5 additional contact hours or 0.5 continuing education units of Boardapproved continuing education that address areas of practice generally related to the particular collaborative practice protocol. This is above the 1.5 CEU that are currently required for all licensed pharmacists.
7.The pharmacist must notify the Board of Registration of Pharmacists of change of employment supervising physician or departure from the state within 30 days. Whenever an authorized pharmacist participating in CDTM is disciplined by the Board whether by agreement or Board order or otherwise subject to any practice restrictions the authorized pharmacist must provide written notification of such discipline or practice restriction to each supervising physician.
8.A supervising physician or physician designee may permit a pharmacist to approach patients of all ages and with all types of conditions to perform the following tasks pursuant to a written CDTM agreement including but not limited to:
a.Collect and review patient history including a medication history and relevant disease state history to manage CDTM services as appropriate per protocol.
b the ambulatory setting monitor vital signs as appropriate per protocol and record pertinent data.
c.Assist the supervising physician or physician designee in an inpatient setting as follows:
i.Record patient progress notes
ii.Order and interpret select laboratory test as outlined in the specific CDTM protocols
iii.Order medications as outlined in the specific CDTM Protocols
iv.Instruct and counsel patients on the use of prescribed drug therapy.
9.PROHIBITED SERVICES BY AN AUTHORIZED PHARMACIST SPECIFIC TO CDTM
a.Pharmacists may not diagnose
b.Pharmacists may not order anything outside of the written protocol
c.Pharmacists may not provide anesthesia procedural sedation or perform procedures involving ionizing radiation
d.Pharmacists shall not be utilized as sole medical personnel in charge of emergency services outpatient services or any other clinical service where licensed physicians are not readily available.
e.Pharmacists may not order radiology tests
