Pharm D or BS in Pharmacy required with current Massachusetts license in Pharmacy.

The Performance Analysis & Improvement Unit (PAI) develops and implements populationbased efforts to monitor and improve clinical effectiveness through systemsbased strategies and interventions. Its goal is to improve the quality and cost effectiveness of health care by informing clinical decisionmaking processes changing patient and clinician behaviors enhancing the care choices of physicians other providers and patients and optimizing care tools and systems.
The Clinical Pharmacist Project Manager works with the Program Director MGPO Pharmacy Quality and Utilization Program to develop implement and manage projects that support Massachusetts General Physicians Organization (MGPO)/Massachusetts General Hospital (MGH) pharmacy quality and utilization programs related to Accountable Care Organization (ACO) and commercial risk contracts. Develops project materials contacts and supports participating patients and coordinates with and provides guidance to prescribing physicians. Assists in obtaining and analyzing drug information analyzes the clinical appropriateness and costeffectiveness of drug utilization provide counter detailing and the dissemination of relevant drug information. Supports chronic disease management efforts in the Performance Analysis and Improvement (PAI) group within diabetes hypertension and hyperlipidemia. Coordinates efforts with staff within MGH Partners Community Physicians Organization (PCPO) and across Partners Healthcare System.
Responsible for the development implementation and of select projects within the Outpatient Pharmacy Utilization Program and other projects to support MGPO/GH quality and utilization initiatives in particular medical management activities under payer contracts through network and specific to MGH.
Addresses high cost drug utilization trends with appropriate programs to encourage clinical appropriateness and costeffectiveness.
Under the direction of the Program Director MGPO Pharmacy and Associate Medical Director meets with targeted subgroups of MGPO physicians to present relevant clinical information (academic detailing) and the formulary compliance process.
Encourages medication adherence by providing education and appropriate intervention for MGPO PCP Specialists Nurse Practitioners and Physicians Assistants.
Design targeted education for specific drug classes.
Participates in chart reviews necessary to support chronic disease management programs.
Supports activities associated with electronic prescribing. Coordinates education and training program for clinicians office staff and relevant others on the electronic prescribing system as needed.
Works closely with the MGPO Medical Director and Associate Medical Directors to support the activities of the MGH and MGPO Medical Directors and Administrative leaders.
Attends and participates in meetings and committees within the Hospital PCHI and Partners Healthcare System to keep abreast of current issues as they affect the MGH/MGPO.
