The Leukemia Programnurse acts as a coordinator and facilitator of thecare for all Leukemia patients referred intothe Leukemia Program. This nurse helps toestablish and coordinate a multidisciplinaryand collaborative Program with social work physicians nurse practitionersand inpatient and infusion nursing staffthatis centered on the care of theleukemia patient. This is ablended role that involves patient educationcare coordination between inpatient and ambulatorycare settingsas well as direct patientcare and patient navigation.
Program Development
- Work collaboratively with physician nursing social work and other staff to develop multidisciplinary team approach to coordinated patient care.
- Assist in developing hoteling program for leukemia patients where appropriate.
- Works with multidisciplinary team to develop educational materials and programming
Coordination of Care
- Collaborate and coordinate with providers to meet needs of this unique patient population.
- Works with Access Nurse to meet all new patients on first visit.
- Facilitates appropriate decisionmaking with providers to ensure that patients receive complete and comprehensive care.
- Effectively communicate productively with patients families multidisciplinary team support staff and inpatient team.
Nursing Skills andAssessment
- Triage and assessment of urgent phone calls
- Initiates telephone followup with patients at regular intervals
- Assess for education and supportive needs for leukemia patient population as appropriate
- Provides direct patient care as appropriate including but not limited to line care lab draws and medication administration.
- Collaborates with physicians and providers to implement and manage plan of care. May include scheduling reviewing results and symptom management.
- Considers patients total wellbeing and recommends appropriate referrals for the patient and family (i.e. social service nutrition and other services)
- Provides ongoing care to the leukemia patient throughout the care continuum.
- Develops and implements teaching/information material for the leukemia Center
- Educates patient and family regarding therapies and supportive modalities
- Develops and provides educational programs/workshops/seminars for patients and families.
- Documents relevant patient information in the LMR record
- Works collaboratively with inpatient teams to ensure smooth transition of care
- Perform injections monitor lab values per protocol
