We are seeking a compassionate and organised Care Coordinator to join our team in Hexham United Kingdom. As a Care Coordinator you will play a crucial role in ensuring the seamless delivery of highquality care services to our clients whilst supporting our dedicated care professionals.
- Develop and maintain efficient schedules for clients and care professionals considering factors such as travel time holidays and lastminute changes
- Match care professionals to new clients arranging introductions and ensuring compatibility
- Liaise with the client services team to ensure client needs are met and schedules are consistent where possible
- Build and maintain positive relationships with both clients and care professionals
- Collaborate with the recruitment team to ensure adequate staffing levels are maintained
- Conduct care consultations risk assessments and develop comprehensive care plans
- Carry out quality assurance visits to ensure service standards are met
- Participate in oncall duties during weekdays and one weekend in four
- Maintain accurate client and care professional information in the electronic scheduling system
- Ensure compliance with equality diversity and equal opportunities policies
- Adapt to changing circumstances and resolve scheduling conflicts efficiently
- Contribute to the continuous improvement of care coordination processes
Qualifications :
- Proven experience in a scheduling role within home care or a relevant environment such as logistics
- Proficiency in IT systems including Microsoft Office or Google Suite
- Experience with CRM software and ability to adapt to new technologies
- Excellent communication skills both verbal and written
- Strong organisational and prioritisation skills with the ability to work calmly under pressure
- Demonstrated ability to work collaboratively in a team environment
- Keen attention to detail and ability to multitask effectively
- Logical and analytical thinking with problemsolving skills
- Selfmotivated with the ability to work independently and meet deadlines
- Knowledge of healthcare regulations and standards in the UK
- Understanding of care planning and risk assessment processes
- Empathetic approach with a genuine passion for providing highquality care
- Resilience and adaptability in managing changing priorities
- Valid UK driving licence (for potential client visits)
- Relevant qualifications in healthcare or social care (desirable)
