Were seeking a dedicated Social Worker / Clinic LMSWto continue the growth of our Social Worker Team at Emory Northlake. As part of our team you will be responsible for independently assessing coordinating and implementing a timely safe patient discharge plan to the appropriate level of care. You will participate in interdisciplinary aproach to patient care management. You will work with complex patient populations who have high acuity chronic illnesses by developing treatment plans and establishing family educational services as a professional resources.
Other desirable transferable experience: solid organ transplant experience hospice experience case management experience with community agencies or medical clinics (sickle cell hemophilia HIV DFCS/foster care with medically fragile children dialysis etc)
Less ideal and less transferable experience: only having acute inpatient psychiatric (Peachford Grady Psych Ridgeview) only having counseling experience MSW only
