The employee in this position shall report to the Communication Center Coordinator. The employee will serve as the programs first point of contact and coordinate the admissions process from the initial request for admission to the final authorization. The responsibilities include screening of potential clients answering phone calls tracking housing availability and documentation in compliance with Volunteers of America Delaware Valley procedures and funding source requirements.
SCOPE OF DUTIES:
- Interact (verbally and nonverbally) with all clients staff and community members professionally with respect and courtesy.
- Maintain client and staff confidentiality in compliance with administrative policies and procedures.
- Attend and participate in regularly scheduled staff meetings supervisory conferences training sessions and meetings as required.
- Disseminate information regarding services provided by the agency to the community.
- Coordinate comprehensive intake assessment procedures on all engagements.
- Communicates special needs and represents clients interests to program staff.
- Network with other community agencies to develop resources for clients.
- Monitor and document client progress according to funding source standards and/or operational requirements in real time.
- Fulfill obligations of training agreements entered into for core curriculum programs.
- Develop appropriate discharge plans for all assigned clients as per contractual requirements and policy and procedures.
- Complete necessary reports and caserelated paperwork in a timely fashion.
- Provide practical assistance for clients by mentoring advocacy coordination individualized support problem solving and direct assistance to help clients obtain housing.
- Assist in maintaining communication between all staff members and clients.
- Work with IMPACT teams case managers social service agencies and community partners to identify and resolve the barriers for obtaining and maintaining housing.
- Coordinate efforts with county and all other organizations who are working toward permanent placement of homeless individuals and/or families.
- Complete other duties as assigned.
ADA ESSENTIAL FUNCTIONS:
- Regularly required standing walking sitting using hands to handle or feel reach with hands and arms climb stairs talk hear and smell.
- Regularly lift and move up to 25 lbs.
- Ability to see clearly at 20 inches or less.
- Ability to identify and/or distinguish colors.
- Ability to observe an area than can be seen up and down or to the left and right while remain fixed on a given point.
