PRACTICE SPECIFICS
Join a growing wellbalanced diverse practice with various levels of acuity and various settings including post acute care longterm care NP supervision and shared care home visits including assisted living and memory units memory assessment clinic with potential expansion of practice and hospital consults.

REQUIREMENTS
- BC/BE in Gerontology or BE/BC in IM or FM with extensive Geriatric training/experience (e.g. fellowship nursing home medical director CMD status from AMDA hospice/palliative care certification)

COMPENSATION
- $271192 for a 1.0 FTE Hired candidates may be eligible to receive additional compensation in the form of bonuses quality incentives or productionbased compensation.
