JOB DESCRIPTION
The Mental Health Care Manager is a key member of both the North Shore Medical Center (NSMC) Mental Health and the integrated Care Management Program (iCMP) Care Management teams who provides and oversees the provision of psychiatric psychosocial and overall mental health services for selected patients and families in the integrated Care Management Program at North Shore.
Upon referral from the iCMP Care Manager the Mental Health Care Manager is involved in assessment and triage of patients and families to ensure provision of appropriate timely and effective evaluation. Depending on the issues and needs presented in the case this initial clinical evaluation may be conducted by the Mental Health Care Manager independently or in collaboration with other members of the iCMP Care Management and Mental Health Team. The Mental Health Care Manager collaborates with the team and communicates relevant information. Treatment plans are prepared in a collaborative fashion as well.
In some cases and as appropriate the Mental Health Care Manager may provide direct treatment/intervention to patients and families. In other cases the Mental Health Care Manager may work with the treating clinicians in psychiatry psychology or other disciplines within and outside of our system helping to ensure that treatment is focused and effective.
Finally the Mental Health Care Manager collaborates with the team in creating and improving the system of care used in the project in determining which interventions have been most helpful for this population and in outlining the essential elements of these interventions so the team can develop a model that may be used by others.
RESPONSIBILITIES
Upon referral from the iCMP Care Manager provides assessment of patients to evaluate mental health/psychiatric history/emotional issues/coping style understanding of illness/adjustment/compliance barriers to care cultural issues abuse neglect and domestic violence.
Working with iCMP Care Manager and other members of the iCMP Mental Health Team as appropriate prepares an intervention plan and disposition.
Provides psychosocial assessment of families to determine: family relationships/systems as they relate to care of the patient.
Identifies family decision makers and caregivers; family understanding of illness and trajectory of care.
Identifies family coping style family resources and cultural issues.
Employs a range of clinical interventions such as individual group or family counseling.
Provides caregiver/family counseling/support to promote family cohesiveness to provide care to patient and prepare families for end of life.
Advocates on behalf of patients and families to gain access to services and resources. Refers patients to other providers as necessary.
Provides mandated assessments when abuse is suspected (child disabled adult elder and safety assessment when violence is reported.
Files reports as indicated by guidelines.
Works effectively as part of the interdisciplinary health care team. communicating regularly with the CMP Care Manager and other members of the iCMP Team on cases and as issues arise.
Coordinates family/team meetings as needed and when appropriate. Provides psychosocial consultation on patient care planning and patient/family management.
Documents timely and relevant information as appropriate in the patient electronic medical record (LMR) Care Management Documentation System (MCCM) and/or other relevant databases and communicates pertinent information to the iCMP Case Manager in a timely fashion.
Acts as a resource to staff and works on a case by case basis to coach and mentor on techniques and approaches to management of psychosocial issues in a high risk population.
Presents and/or discusses clinical work in formal and informal case reviews and seminars as indicated.
May participate in research projects.
May initiate/implement psychosocial programs based on patient/family identified need as indicated by the iCare Management Program.
Programs may be intermittent/informal or ongoing.
Performs other duties as assigned.
