St. Pauls Hospital Mental Health and Emergency Departments
Reporting to the designated Social Work Site Leader and receiving guidance and support from the Professional Practice Leader and/or Research Education and Practice Coordinator this position supports the interdisciplinary program/team under the direction of the Patient Care Manager in achieving program patient and resident goals.
Responsibilities:
Provide therapeutic clinical counselling services
Conduct comprehensive biopsychosocial assessments
Maintain patient/resident records
Provide consultative and direct services to patients/residents families staff and community health professionals regarding community resources
Lead the interdisciplinary team in the discharge planning process
Coordinate clinical/case conferences
Develop and deliver inservices and presentations for staff
Participate as a member of local committees and group meetings
Maintain broad knowledge of developments in the Social Work profession
Education:
Masters in Social Work from an accredited university or a Bachelor of Social Work degree from an accredited School of Social Work with an equivalent combination of education training and experience.
One year of recent related experience in a designated program area or an equivalent combination of education training and experience.
Current full registration with the British Columbia College of Social Workers.
Skills and Abilities
Your Day to Day
Provides therapeutic clinical counseling to patients families and groups by identifying problems personal concerns and treatment objectives and facilitating change in coping styles behaviors and attitudes. Coordinates with interdisciplinary teams and makes referrals to appropriate resources.
Conducts comprehensive social work and psychosocial assessments gathering relevant data regarding family environment culture living situation and financial status to inform and impact treatment plans.
Maintains patient/resident records documenting assessments progress notes and reports in accordance with standards policies and regulatory requirements for future reference.
Contributes to discharge planning by assessing care needs eligibility and service requirements. Collaborates with community partners and hospital services to develop and coordinate discharge plans for patients/residents with complex needs.
Provides consultative services to patients families staff and community professionals regarding community resources including financial assistance and resource opportunities.
Participates in interdisciplinary care team conferences for patient/resident reviews admissions and discharges.
Supports patients families and teams in understanding legislative processes related to Health Care Consent and Care Facility Act Medical Assistance in Dying Mental Health Act and Adult Guardianship Act.
Provides ongoing program evaluation from a social work perspective contributing to care issues and program effectiveness to ensure proper treatment and care for patients/residents.
Develops coordinates and delivers inservices presentations and training for staff in collaboration with the interdisciplinary team.
Participates in local committees to evaluate programs identify opportunities for improvement assess resources and enhance efficiency and effectiveness in service delivery.
Facilitates and participates in group meetings providing clinical counseling services and psychosocial education as needed.
Maintains broad knowledge of developments in social work through literature review expert consultations and professional development to improve clinical practice and patient care.
Maintains records such as workload statistics analyzing trends and recommending action plans to improve service delivery.
Develops research protocols and conducts social work interdisciplinary research and special projects ensuring clinical practices align with current research findings.
Provides teaching and supervision to social work students guiding them in the treatment of patients/residents.
Performs other related duties as assigned
