What Youll Do in This Role:
- The LifeCircles clinical team delivers primary care palliative care and endoflife care for patients aged 55 and older. This position is perfect for the healthcare provider who wants to be patientcentered creative and collaborative in their approach to care.
- The successful candidate will join an experienced Interdisciplinary Team and work closely with other Advanced Practice Providers and healthcare professionals in a wellsupported teambased environment.
Primary Responsibilities:
- Participating in daily interdisciplinary team meetings
- Completing periodic health assessments
- Assessing and treating acute needs and illness
- Advanced Care Planning
- Patient and Caregiver Education
What Youll Need:
- Current license and certification to practice as a Nurse Practitioner (NP) or Physician Assistant (PA) in the State of Michigan
- Masters degree in nursing or satisfactory completion of a physician assistant program
- BLS certification
- Current DEA registration
- State of Michigan Controlled Substance License
- Minimum of 1year experience with the frail and elderly or longterm care population preferred
