The Hants Health and Wellness Team (HHWT) is a community based integrated chronic disease prevention and management service that provides wellness programming and navigation chronic disease management nutrition education and emotional wellness counselling. The HHWT is an interdisciplinary team that builds community connections and partnerships to provide programs and services that are responsive to community needs.Principles of patientcentered care behaviour change and selfmanagement are core to all programming and services.

Reporting to the Health Services Manager Primary Health Care the incumbent will use critical thinking and clinical judgment supporting patients (in both individual and groupbased settings) with the management of their chronic disease and wellness. The Wellness FacilitatorPhysiotherapist is team focused and will collaborate with HHWT staff primary care providers other health care providers and community groups to maintain awareness of existing supports within the community and surrounding areas. This work is done through a community development population health and collaborative approach with emphasis on selfmanagement supports.

We would love to hear from you if you have the following:

Bachelors degree in physiotherapy from an accredited university required
Membership with the Nova Scotia College of Physiotherapists required
Must be eligible for membership with the Canadian Physiotherapy Association
Minimum one year experience in each of the following: working with people with chronic disease in an exercise program or setting; working with community groups/partners building trusting relationships through working together on projects/initiatives/partnerships that benefit the public (professional and/or volunteer); Individual and group education/facilitation utilizing adult education selfmanagement supports and motivational enhancement principles
Understanding of population health health promotion strategies and behaviour change principles
Indepth knowledge of local health and communitybased services.
Demonstrates personcentered/familyfocused diversity focused approaches to care
Welldeveloped interpersonal teamwork oral and written communication skills
Demonstrates flexibility adaptability and willingness to change

Demonstratesinnovation in program planning implementation and evaluation and quality improvement planning skills
Computer skills (MS Office email and internet)
Annual BLS AED certification and WHMIS training and certification

RQI training
Travel within the community is required with valid Nova Scotia drivers license and daily access to a reliable vehicle.
Competencies in other languages preferred; French an asset
