SUMMARY
Float Pool Social Worker
In collaboration with patients/families case managers physicians and the interdisciplinary team the Social Worker I provides direct psychosocial and practical care including social work assessments problem identification goal setting behavioral intervention and implementation of care plans. The Social Worker collaborates with all disciplines to optimize patient outcomes; scope of services may include but is not limited to: developing action plans to mitigate psychosocial barriers to discharge; complex postacute care coordination; patient/family engagement and education; and employing clinical interventions (to address immediate needs.

Salary Range: Minimum28.56per hour Maximum47.17per hour.Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
