Job Description
Essential Functions:
- Adhering to the Best Practices established by the Board of Nursing and CAHC Accreditation Standards
- Open communication with other Team Members Clients and Family
- Provide input regarding placing certified home health aides (CHHAs) appropriately with clients
- Supervision of CHHAs in the greater community
- Making monthly followup calls/visits to ensure adherence of Care Plan
- Evaluate effectiveness of care provided by CHHAs with timely completion of 60day skills assessments during onsite visit
Qualifictions:
- New Jersey Registered Nurse (RN) License BSN preferred.
- BLS Certification
- New Jersey Drivers License and an automobile in good working condition with appropriate insurance coverage in accordance with New Jersey laws.
- Minimum of oneyear supervisory experience in health care setting preferably home care hospice or longterm care
- Ability to join oncall rotation
