POSITION TITLE: Transition Specialist DEPARTMENT: Special Education
REPORTS TO: Director of Special Education


POSITION SUMMARY: The Transition Coordinator will provide coordination and facilitation of transition services/programs as it relates to social academic and vocational growth of students with disabilities. The Transition Coordinator will work directly with the Director of Special Education and collaboratively with Process Coordinators to evaluate the effectiveness of the networks programs based on postsecondary tracking of students who graduate from the network.
ESSENTIAL JOB FUNCTIONS: include the following. Other duties may be assigned
Communicates transition procedures to staff.
Function as a resource to classroom teachers which may consist of coteaching on various subjects such as independent living skills employability skills etc.
Help initiate/coordinate inservice training activities related to transition
Serve as the liaison between the school and postsecondary agencies.
Participate as a member of the IEP team when discussing transition services.
Coordinate transition activities for the network
Ensures for each student beginning not later than the first IEP to be in effect when the child reaches 16 years of age will have an appropriate transition plan that will reasonably enable the students to meet the postsecondary goals.
Ensure all students have measurable postsecondary goals that cover education or training employment and as needed independent living.
Monitors student transition plans for compliance.
Maintain good communication and strong working relationship with other school staff students and parents.
Inform families of community resources
Attend meetings and training sessions related to transition as deemed appropriate by the Director of Special Education.
Maintains confidentiality and nonjudgmental views of children and families.
Perform other duties as assigned by supervisor


QUALIFICATION REQUIREMENTS: To perform this job successfully an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge skill and/or ability required. Education/Experience: Bachelors degree in Special Education. A minimum of 3 years of teaching experience in related area of concentration.

Certification: Valid Teaching Certificate in Special Education from the Missouri Department of Elementary and Secondary Education or possess appropriate credentials to prove eligibility for Missouri certification.

Language Skills: Ability to read analyze and interpret general business periodicals professional journals technical procedures or government regulations. Ability to write reports business correspondence and procedure manuals. Ability to effectively present information and respond to questions from groups of mangers clients customers and the general public.


Reasoning Ability: Ability to define problems collect data establish facts and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
Other Skills and Abilities: Must be able to transport between school buildings and cities. Ability to apply knowledge of current research and theory in specific field. Ability to establish and maintain effective working relationships with students staff and the community. Ability to communicate clearly and concisely both in oral and written form. Ability to perform duties with awareness of all Confluence requirements and Board of Education policies. Ability to utilize technology in establishing appropriate record keeping program for students programs and staff.

While performing the duties of this job the employee is regularly required to sit and talk or hear. The employee is occasionally required to stand and walk. Specific vision abilities required by this job include close vision and the ability to adjust focus. The position requires meeting deadlines with severe time constraints interacting with the public and staff irregular or extended work hours. The employee is responsible for safety wellbeing and work output of others. Must be able to meet demands from several people.
