Role Overview:
Support Coordinator:
To support people with psychosocial disability to participate more fully in the community and enhance their capacity for social and economic participation through:
- Strengthening their ability to coordinate and implement supports in their National Disability Insurance
- Scheme (NDIS) individualised funding packages
- Supporting their engagement with mainstream formal and informal services
- Developing capacity and resilience in their network.
Recovery Coach:
- Developing a recoveryenabling relationship with people including their family and carers
- Supporting a person to develop and implement their individual recovery plan
- Coaching a person to increase recovery skills and personal capacity including motivation strengths resilience and decisionmaking
- Collaborating with broader systems including available services and resources
- Supporting people to become active participants in the NDIS
Requirements:
- Minimum of a Cert IV in a Community services related discipline (ideally a completed degree or Diploma)
- Experience in a human services environment particularly experience in understanding service delivery and how it can best deliver for clients with persistent and complex mental health.
- Experience in assisting participants to test their eligibilityof the National Disability Insurance Scheme (NDIS) or transition them to other appropriate supports.
- Highlydeveloped communication interpersonal and negotiation skills including the ability to work and communicate effectively with clients their families community agencies and other professionals.
- Ability to meet deadlines in a high demand work environment and to exercise judgment and initiative.
- Excellent organisational and time management skills to hold a caseload of up to 25 participants
- Ability to work independently and within the team
- Must be eligible to apply/currently hold a WWCC
- Must hold/be willing to apply for NDIS worker screening clearance
- Current Drivers License
