Clinical Pharmacist Specialist - Emergency Medicine

POSITION SUMMARY:

The Emergency Medicine (EM) Clinical Pharmacy Specialist (CPS) is responsible for the provision of comprehensive clinical pharmacy services for the ED and all associated areas including pharmacokinetic and therapeutic consultation.

The emergency department has EM CPS coverage 24/7 with the exception ofon weekends and holidays. EM CPS provide clinical coverage during the shifts ofand. The EM CPS will rotate betweenandin addition to every 4th weekend and one to two holidays per year (combination of major and minors) based on department need. The EM CPS will rotate through covering vacation weeks of 3rd shift ED coverage per year based on departmental policy.

In addition to the primary job duties listed above the ED pharmacist may be crossedtrained to assist with additional duties in the department. Such duties may include providing clinical pharmacy coverage in the critical care unit or other clinical pharmacist positions and other duties listed below.

MINIMUM QUALIFICATIONS:

Education: BS Pharm or Pharm D

Required length and type of experience: Three 3 years of emergency medicine experience

Required licensure certification or registry:

• Must be licensed or eligible for licensure as a registered pharmacist in Ohio
• BCEMP (Board Certified Emergency Medicine Pharmacy) preferred
• PGY1/PGY2 residency preferred
• Maintains BLS ACLS and PALS Certification
• Maintains OARRS access
• Immunization certification

Below are the duties and responsibilities for this position.

Provision of comprehensive pharmaceutical care to Emergency Department patients drug distribution support clinical intervention precepting teaching and pharmacy representation in appropriate emergency department and hospital committees.

Clinical Medication Management Role:

• Review patient charts including prospective medication order verification for accuracy and appropriateness.
• Participate in the drug therapy management of medical emergencies (i.e. code trauma stroke rapid sequence intubation conscious sedation sepsis etc..
• Maintain expertise in emergency medicine to collaborate with providers to optimize pharmacotherapy regimens utilizing current evidencebased recommendations including drug selection dosing route of administration and monitoring.
• Provide care under ED collaborative practice agreements in culture evaluation and injectable medication administration.
• Dispense apixaban or rivaroxaban for the treatment of venous thromboembolism or atrial fibrillation.
• Provide rabies vaccinations to patients on follow up rabies encounters.
• Dispense naloxone kits through ODH Project DAWN Program.
• Provide bedside preparation of tenecteplase prothrobmin complex concentrate and other emergent medications.
• Document clinical interventions in an appropriate and timely manner.
• Prevent detect monitor document and report adverse drug reactions and medication errors.
• Provide patient specific and general drug information to providers nursing patients and other healthcare professionals.
• Provide active pharmacy representation on various ED and hospital committees including development and maintenance of policy & procedures and clinical pathways.

Clinical Cost Containment Role:

• Promote cost avoidance and cost saving measures in addition to adhering to SWG formulary.
• Converting nonformulary orders to formulary when possible but coordinate procurement of nonformulary when necessary.
• Promote and demonstrate rational costeffective drug therapy.
• Develop and conduct target drug programs and medication use evaluations as needed and report results to the Pharmacy and Therapeutics Committee or Subcommittees.
• Document clinical consultations and cost avoidance as part of the departmental clinical documentation program.
• Evaluate medications stocked in automatic dispensing cabinets for appropriateness and recommend changes based on clinical/financial needs.
• Anticipate changes in drug therapy and report the potential impact it will represent to the overall hospital and drug budget.

Medication History/Reconciliation Role:

• Provide continuous quality improvement and quality assurance for medication history training program.
• Provide assistance with medication histories when needed including updating patient profiles with current medications and allergies.
• Review OARRS reports for physicians upon request.
• Serve as a resource for medication history technicians.

Educational Role:

• Provide inservices for emergency department related information to nursing and medical staff.
• Take a leadership role in the facilitation of teaching programs of the Department of Pharmacy Services for pharmacists technicians and students related to emergency medicine.
• Active involvement in precepting for PGY2 EM resident PGY1 residents APPE and IPPE students.
• Provide presentations publications and other informative activities on emergency medicine and other drugrelated topics to the health care community and general public.
• Participate actively in emergency medicine team meetings and conferences.
• Develops and maintains annual competencies related to emergency medicine for pharmacy staff.
• Provide rotating on call service to PGY2 EM resident during weekend staffing.

Operational Role:

• Prospective order verification for assigned areas.
• Resolves drug distribution related issues.
• Resolves automated dispensing cabinet related issues.
• Covers assigned shifts as determined by departmental need.

Complexity of work:

• Requires critical thinking skills requiring decisive judgement and the ability to work with minimal supervision.
