We are looking for a motivated Nurse Practitioner to join our practice in New Haven CT. Our ideal Nurse Practitioner is experienced in orthopedics/musculoskeletal issues comfortable working within an integrated setting collaborating with a knowledgeable and wellrespected team of professionals to offer a comprehensive approach to patients living with chronic pain. Job duties include performing injections so experience doing those types of procedures is a plus.

Duties:
- Patient exam/evaluation (medical history physical exam review pertinent med records/imaging)
- Order and review tests/labs
- Diagnosis and treatment plan formulation
- Patient education as to diagnosis treatment options rehab/exercises and various modalities
- Manage patient care
- Treatments trigger point and injections under US guidance
- Documentation and progress notes daily timely and accurate
- Collaboration with the team and senior providers as needed for comprehensive patient care plans
Requirements:
- Graduation from an accredited Nurse Practitioner (NP) program
- NP license in CT
- Minimum of 3 years clinical experience as a Nurse Practitioner
- Orthopedic Physical Med and Rehab or Pain management background is preferred
- Experience with trigger point and joint injections preferred
- Spanish speaking a plus but not required
Schedule:
- Full time (Monday Friday) 8:00am4:30pm
Compensation:
- $125k$130k annually (DOE) plus potential bonus compensation
