Your Role as a Behavioral Health Clinical Team Manager

As a key leader youll ensure seamless highquality home health services while building and inspiring a team of compassionate healthcare providers. You will:

• Oversee patient care services including staffing care coordination and referrals.

• Ensure patient care starts promptly following regulatory guidelines.

• Supervise and support your team to meet patients evolving needs through personalized care plans.

• Coordinate and communicate effectively with patients families physicians and other care providers.

• Monitor clinical outcomes update care plans and ensure compliance with physician orders.

• Drive quality improvement and ensure your team meets or exceeds clinical standards.

What You Bring to Elara Caring

Were looking for leaders who are passionate about making a difference. To succeed in this role youll need:

• Associates Degree in Nursing (BSN preferred).

• 2 years of home health or behavioral health experience as a Registered Nurse.

• 1 year of supervisory experience in a healthcare setting.

• Active state Registered Nurse license.

• Proficiency in enterprise systems like Workday and HomeCare HomeBase and familiarity with MS Office Suite (Word Excel Outlook).
