Job Description

Position Summary:The Ohio Healthy Transitions Program (OHTP) coordinator is expected to support the activities directly related to improving access to treatment and support services for youth and young adults ages 1625 who have a serious emotional disturbance or serious mental illness . This position will be involved with increasing awareness screening and detection outreach and engagement referrals to treatment coordination of care and evidenceinformed treatment.

Responsibilities Include:

  • Participate and lead outreach to Transitional Aged Youthand their families; serve as an ambassador of the youthand their families to navigators and programs
  • Work with individuals and groups in the community to determine the needs of youth and young adults.
  • Identify the needs and interests of the youth and young adults and find resources and organizations to help them
  • Establish an advisory council across youth and adult serving systems that is inclusive of key decisionmakers.
  • Maintain high standards of ethical and professional conduct and adhere to Agency policies and procedures.
  • Travel from our Summit andMedina County locations

Qualifications:

  • Must reside in Summit or Medina county
  • Minimum bachelors degree in Social work psychology nursing or related field
  • Demonstrated knowledge of the local Mental Health system Juvenile Court System or Department of Child and Family systems
  • Model competence to their clients through the candidates own lived experiences with the above listed entities
