Rutgers The State University is seeking a Habilitation Counselor in the Department of Community Outreach Services within Rutgers University Behavioral Healthcare. The primary purpose of the Habilitation Counselor position is to provide case management services to individuals diagnosed with mental health issues age 18 years and over who are incarcerated at the Middlesex County Adult Correction Center and are ready for reentry into the community. Among the key duties of this position are the following: Provides referral linkage transportation and support to assigned individuals served. Conducts assessments in the county correctional facility and provides effective planning for release as indicated by supervisory observation and record review. Maintains clinical documentation standards as mandated by UBHC policy and other applicable accreditation and certification entities. Completes written assessment progress notes and transfer/termination summary in individuals record in an effective and timely manner within program guidelines as evidenced in record reviews. Assumes shared case management responsibilities as needed for JIS JAG team; acts as a liaison with collaterals and community agencies as appropriate for planning and linkage. Demonstrates ability to establish and maintain an effective working relationship within the team and among agencies and providers. Maintains knowledge and trends in the field. Regularly attends and participates in quarterly JIS JAG meetings called by DMHS . Regularly attends and participates in workshops and other training events to enhance existing skills as documented by attendance at these activities. Understands and adheres to Rutgers compliance standards as they appear in RBHSs Corporate Compliance Policy Code of Conduct and Conflict of Interest Policy.
Overview
Rutgers University Behavioral Health Care UBHC established in 1971 offers a full continuum of evidence based behavioral health and addiction services for children adolescents adults and seniors throughout New Jersey. UBHCs 1060 experienced behavioral health professionals and support staff are dedicated to treatment prevention and education. UBHC one of the largest providers of behavioral health care in the country has a budget of $260 million and has 15 sites throughout New Jersey. Services are readily accessible and include: inpatient outpatient partial hospitalization screening crisis stabilization family/caregiver support community outreach and case management supportive housing supported employment prevention and consultation employee assistance programs and a licensed therapeutic school from preschool through high school. Specialty services include the New Jersey suicide prevention helpline and peer help lines for police veterans active military teachers mothers of special needs children and child protective service workers. In FY2016 UBHC treated 16199 consumers had 24502 admissions and touched the lives of 19441 individual callers through peer support. In addition UBHC is the primary mental health training resource for the New Jersey departments of Human Services Children and Families and Corrections delivering 16000 trainings each year.
