Job Description

Responsible for allcoordination ofactivities and overall facilitation of clinical flow ofpatientthroughout the course of coordination of a patients care while in residential services including integration with family services sessions communication with referral sources assessment of patient progress and communication to the clinical team regarding special treatment needs and procedures. 

Responsibilities

The duties listed below reflect the general details considered necessary to describe the principal functions of the job as identified and shall not be considered as a detailed description of all work requirements that may be inherent in the position. 

  • Completion of patient assessments such as bio psychosocial assessment patient goals and treatment planning. 
  • Utilize multi-disciplinary treatment team to determine appropriate level of care and identify behavioral health needs that may require external placement outside of Brookdale Premier Recovery. 
  • Facilitate group therapy and individual counseling. 
  • Recognizing assessing determining and evaluatingsymptoms of addictions according to ASAM and DSM V criteria. 
  • Work with family therapist to assess family dynamics recommend family education programs and ongoing family therapy. 
  • Work with Patient Experience Managers to develop a comprehensive continuing care plan that includes input from clinical referents internal referents multi-disciplinary treatment teams family and patient. 
  • Work within the ethical constraints of a certified addiction counselor and other professional or organizational ethical standards as they apply to the position. 
  • Document treatment plans progress notes treatment plan updates case consultations phone calls and discharge summaries in the patient medical record in accordance with Brookdale Premier Addiction Recovery policies and procedures. 
  • Complete necessary utilization review information. 
  • Contacts oversees and coordinators all communicationinvolvingpatientsreferentand family as appropriate in assessment and care planning processes. 
  • Conducts individual and group counseling sessions congruent with 12-step program to motivate and support patient awareness understanding and motivation to change related to addiction and recovery issuesand is responsible for fully integrating the process into thepatientstreatment plan. 
  • Communicates and workseffectively with a team consults with the multi-disciplinary team patientfamilyand referent (consistent with CFR 42 and CFR 45) to address clinical issues both verbally and in writing related to the individualized treatment plan continuing care including types of care level of care. 
  • Coordinatescommunication and collaboration with internal and external customers( business development officers). 
  • Must be ableto develop and present psychoeducational lectures specialty groups relative to the disease of addiction relapse behavioral addictions and 12 step recovery. 
  • Regular and predictable attendance is an essential function of the job. 

This is a full-time position that will work Sunday-Thursday.

Qualifications :

QUALIFICATIONS:Clinical Counselor I 

  • Associates orBachelors Degree required in Human Services or other related discipline. 
  • 2 years experience in addiction treatment preferred. 
  • Experience in facilitating group therapy. 

QUALIFICATIONS: Clinical Counselor II 

  • Bachelors Degree requiredin Human Services or other related discipline. Masters degree in human services preferred Licensure preferred. 
  • Specialty training preferred (MI DBT Art Therapy MBSR CPT CBT). 
  • Experience in lifespan developmental issues. 
  • Experience in facilitating group therapy. 
  • CADC preferred. 

QUALIFICATIONS: Clinical Counselor III 

  • Masters degree inHuman Services or other related discipline required. 
  • Specialty training preferred (MI DBT Art Therapy MBSR CPT CBT). 
  • Experience in lifespan developmental issues. 
  • Experience in facilitating group therapy. 
  • CADC Preferred.
