Job Title:Advanced Practice Provider Nurse Practitioner or Physician Assistant Whole Person Pain Service/ APP Sr. Instructor

Job Summary:

The Advanced Practice Provider (APP) will provide care within the Department of Family Medicine serving both the Whole Person Pain Service (WPPS) and as an APP in a family medicine clinic.

The Whole Person Pain Service (WPPS) will provide holistic chronic pain care for primary care patients established in the department of family medicine. The service will be 100 virtual and will serve as the chronic pain hub for patients in the department who are currently prescribed chronic opioid medications. The Advanced Practice Provider in this role will be responsible for two main functions: chronic pain patient care and care coordination.

The APP will also continue to provide coverage at their current UCHealth/Department of Family Medicine clinic location. The APP will provide fullspectrum ambulatory care including but not limited to chronic disease management acute and urgent care and wellness and preventative care for adult and pediatric patients.

The Advanced Practice Provider (APP) evaluates assesses diagnoses prescribes and educates patients regarding their particular illness or chronic disease. The position provides highquality primary care to all patients evaluates and treats acute and chronic problems for the patient population completes histories thorough assessments and physical examinations on all patients with accurate and complete documentation reflected in medical records and collaborates and communicates with other departments and colleagues such as the pharmacy to ensure optimal patient care.

This position is a fulltime 1.0 FTE position.

The duties and responsibilities of the position include but are not limited to:

Whole Person Pain Service Clinical/Direct Patient Care 0.200.25 cFTE)

  • Provide patient care to patients with chronic pain in a virtual setting
  • Perform intake routine followup and group visits for patients on chronic opioids
  • Participate as needed in chart review and clinical coding compliance activities
  • Collaborate as appropriate with medical staff and specialists in the care of complex patients
  • Interact with other health professionals to create guidelines and coordinate patient care
  • Triage the WPPS in basket and telephone line and support patients as needed
  • Using opioid registries proactively outreach patients and/or PCPs who may benefit from the WPPS
  • Using opioid registries determine the highest risk patients in need to be discussed at the collaborative care conferences

Whole Person Pain Service Care Management 0.30.50 FTE)

  • Address in basket and phone messages and providing triage when appropriate
  • Opioid registry management with direct patient outreach
  • Highrisk patient case conference coordination

Float APP Clinical/Direct Patient Care 0.250.50 cFTE)

  • Provide quality accessible costeffective primary care for patients served by University of Colorado Hospitals and other DFM sites as designated
  • Provide direct patient care for acute care patients who require/request sameday appointments
  • Collaborate as appropriate with medical staff in the care of complex patients
  • Interact with other health professionals to create guidelines and coordinate patient care
  • Collaborate with other team members in chronic disease management and patient care management
  • Will not participate in afterhours call pool
  • Provide quality care for other populations as assigned
  • Support and collaborate with medical nursing and office staff

Administrative/Professional Development

  • Maintain eligibility for School of Medicine Faculty
  • Keep abreast of the latest developments in field both academically and through applied skills
  • Attend and present at conferences meetings and trainings to maintain certification and enhance professional skills; participate in faculty development activities
  • Accept other responsibilities assigned by the Medical Director

Work Location:

Hybrid this role is eligible for a hybrid schedule with occasional meeting on the Anschutz campus in Aurora Colorado as needed. Interested candidates need to live in Colorado at time of hire.

Qualifications:

Minimum Qualifications:

Applicants must meet minimum qualifications at the time of hire.

Senior Instructor:

  • Masters/Doctor of Nursing practice degree or graduation from an NCCPAapproved physician assistant program
  • Licensed as a Registered Nurse and Nurse Practitioner or Physician Assistant in the State of Colorado
  • Completion of required 1000 hours (provisional prescription authority/mentorship) or other minimum as defined by State of Colorado in order to obtain full prescriptive authority and DEA registration
  • Medicare/Medicaid credentials
  • Unrestricted DEA license and current CPR certification
  • National certification by ANCC or AANP as family or adult nurse practitioner (if FNP)

Preferred Qualifications:

  • Three years (full time) of practice experience working in a Patient Centered Medical Home/Ambulatory Family Medicine and/or Adult Medicine practice preferred
  • Experience working with Epic or another EHR
  • Knowledge and skills necessary to provide care appropriate to the age of the patients served; the ability to assess data reflective of the patients status and interpret the appropriate information needed to identify each patients requirements relative to their agespecific needs and to provide the care needed as described in the offices policies and procedure.
  • Demonstrated commitment to cultural humility and minimizing implicit bias in clinical care

Knowledge Skills and Abilities:

  • Ability to consult with supervisory physician on patient care and administrative issues
  • Ability to document history physical examinations assessments and treatment plans for each patient by using the electronic medical record and enter followup diagnostic testing and laboratory studies that have been ordered
  • Ability to discuss and promote wellness for all patients
  • Knows when and how to refer patients to other health care professionals when the care that needs to be provided is beyond their scope of practice.
  • Working knowledge of HIPAA
  • Ability to prioritize and respond to emergencies in a calm manner
  • Ability to verbalize information/directions clearly and concisely
  • Ability to maintain confidentiality of sensitive information

The starting salary range or hiring range for this position has been established as HIRING RANGE:
