SUMMARY DESCRIPTION OF WORK
The Care Team Specialist works with clients to provide coordination of services of which they are enrolled. They are the point person to ensure clients team both internally and with outside providers is communicating on a regular basis to ensure optimal care. They help refer to other resources or programs when needed. They are responsible for enrollment into programming and ongoing documentation required for the Certified Community Behavioral Health Clinic (CCBHC).
Compensation: wage is based upon total years of relevant experience.
Work Schedule: 37.5 hours per week Monday through Friday 8:30 a.m. to 5:00 p.m.
