Community Integrated Case Manager

CBI is the premiere nonprofit fully integrated healthcare provider of substance use and behavioral health programs in Arizona including prevention education and treatment using cuttingedge nationally recognized evidencebased models.
Job Summary
The Community Integrated Case Manager provides case management services in shelter or housing programs and serves as the liaison between the CBI Shelter and Housing Programs and the CBI PCMH clinic. The Community Integrated Case Manager assists the patient and treatment team in planning connecting outreaching advocating coordinating and monitoring the patients needs and progress while in the program. This position also assists patients in gaining access to health services financial assistance employment education social services and natural supports as well as developing independent living skills. The Community Integrated Case Manager assesses patient needs develops and monitors individualized treatment plans and documents services provided in an accurate and timely manner. CBI employees must embrace the recovery philosophy and promote a safe recovery environment for clients
Skills/Requirements:
Highschool diploma or GED and either:
o 18 credit hours of posthigh school education in a field related to behavioral health and two years of fulltime behavioral health work experience; or
o four years of fulltime behavioral health work experience.
Has an associates degree and at least two years of fulltime behavioral health work experience; or
Has a bachelors degree in a field other than behavioral health and at least one year of fulltime behavioral health work experience; or
Has a masters degree or bachelors degree in a field related to behavioral health.
Minimum of six months of recovery from substance use and/or mental health disorders preferred.
Current AZ Drivers License (valid and in good standing).
Clear 39month Motor Vehicle Record.
Arizona Fingerprint Clearance card (program specific)
