If that describes are made for this!The RN Case Manager will be based out of our Portsmouth NH office and will service the beautiful Greater Seacoast RN Case Manager is responsible for managing a defined caseload of patients providing direct patient care and comprehensive nursing services to individuals in the community. Open positions include Full Time Part Time and Per Diem.
Pay:$36$40/hour
Hours:MondayFriday 8:30am5pm(Flexible days if desired!
Location:Greater Seacoast Area

Job Requirementsof theHome Health Registered Nurse Case Manager:
- Valid Nursing License in the State of New Hampshire
- Nursing experience required
- Current CPR Certification required (We offer monthly renewal classes paid for by Interim HealthCare)
- Reliable transportation
