Job Description
Are you a dynamic customeroriented individual who thrives in a fastpacedimpactful environment Griswold Care Pairing is seeking a Referral Coordinator to Manage VA Cases to join our team! In this exciting role youll play a key part in helping our veteran clients maintain their independence and achieve peace of mind by providing exceptional case management and staffing support.
What Youll Do:
ClientCaregiver Matchmaking: Ensure clients receive the best care by referring caregivers that best match their specific needs.
Relationship Management: Build and maintain strong trusting relationships with both clients and caregivers to ensure satisfaction. Build and maintain strong trusting relationships with the VA staff.
Retention & Introductions: Conduct retention visits and introduce clients to their caregivers.
Assess Client Needs: Perform comprehensive assessments to fully understand each clients needs challenges and objectives.
OnCall Rotation: Participate in oncall duties on a rotating basis to ensure continuous support for clients.

What We Expect From You:
Experience & Education: Associate degree in Human Services or a related field preferred (not required). Relevant experience in home care scheduling or a similar role is a plus.
Strong Communication Skills: Excellent verbal and written communication skills with a pleasant phone manner.
Techsavvy: Proficiency in Microsoft Office and other computer programs.
Organized & DetailOriented: Ability to solve problems creatively stay organized and maintain attention to detail.
Passion for Helping Others: A strong desire to improve the lives of others and contribute to the success of the team.
