The State Care Coordinator (SCC) should have knowledge of the community resources and core competencies to effectively establish relationships in the community build a rapport of trust with the client facilitate substance use disorder recovery planning negotiate conflict resolution and engage and empower clients utilizing evidencedbased motivational interviewing strategies. The State Care Coordinator should promote continuity of care by helping clients to transition from one level of care to another to improve recovery outcomes and to access recovery support services and community resources available within their jurisdiction. The State Care Coordinator shall be in compliance with all state and federal regulations and adhere to all PCS policies and standards of practice maintaining the highest level of professional conduct and ethics.

Education & Experience
High School Diploma or equivalent
Minimum of 2 years experience working and providing case management to individuals that have substance use diagnoses. Preferred experience also with behavioral/mental health diagnoses.
Knowledge Skills & Abilities
Knowledgeable regarding community resources related to substance use mental health and case management.
Clear concise and empathetic communication.
Listening understanding and reflective skills.
Ability to build strong empathetic and accountable relationships with participants to establish a rapport of trust and respect.
Ability to discern an individuals strengths and needs at different stages of their recovery and to connect them with appropriate recovery support services and resources or referral to behavioral health treatment if needed.
Cultural and linguistic sensitivity that promotes diversity equity and inclusion.
Fundamental knowledge of the Stages of Change.
Ability to work independently and prioritize daily schedule to best address the needs of staff and participants.
Valid drivers license dependable transportation and vehicle insurance.
