Job Description
An exciting job role has recently become available at Reigate. They are looking for a new member for Assessments.
Duties:
- Needs Assessments: Evaluate the care and support needs of individuals to determine their eligibility for services under the Care Act 201412.
- Risk Assessments: Identify any risks to the individuals wellbeing and safety and develop plans to mitigate these risks
- Care and Support Planning: Create detailed care plans that outline the support and services required to meet the individuals needs
- Reviewing Plans: Regularly review and update care plans to ensure they remain relevant and effective
- Guidance: Offer information and advice to individuals and their families about available services and support options
- Counseling: Provide emotional support and counseling to help individuals cope with their circumstances
- Crisis Intervention: Step in during crises to provide immediate support and ensure the safety of vulnerable individuals
- Safeguarding: Work to protect individuals from abuse neglect and exploitation
- MultiAgency Work: Coordinate with other professionals and agencies such as healthcare providers to ensure a holistic approach to care
- Referrals: Make referrals to other services and agencies as needed to provide comprehensive support
- Documentation: Keep detailed records of assessments care plans and any interventions or support providedeporting: Write reports and maintain accurate documentation to support decisionmaking and ensure accountability

What we are looking for in a Social Worker:
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK licence
- Postqualified experience
- Degree level or equivalent in Social Work
