Job Description

An exciting job role has recently become available at Reigate. They are looking for a new member for Assessments.

Duties:

  • Needs Assessments: Evaluate the care and support needs of individuals to determine their eligibility for services under the Care Act 201412.
  • Risk Assessments: Identify any risks to the individuals wellbeing and safety and develop plans to mitigate these risks
  • Care and Support Planning: Create detailed care plans that outline the support and services required to meet the individuals needs
  • Reviewing Plans: Regularly review and update care plans to ensure they remain relevant and effective
  • Guidance: Offer information and advice to individuals and their families about available services and support options
  • Counseling: Provide emotional support and counseling to help individuals cope with their circumstances
  • Crisis Intervention: Step in during crises to provide immediate support and ensure the safety of vulnerable individuals
  • Safeguarding: Work to protect individuals from abuse neglect and exploitation
  • MultiAgency Work: Coordinate with other professionals and agencies such as healthcare providers to ensure a holistic approach to care
  • Referrals: Make referrals to other services and agencies as needed to provide comprehensive support
  • Documentation: Keep detailed records of assessments care plans and any interventions or support providedeporting: Write reports and maintain accurate documentation to support decisionmaking and ensure accountability

What we are looking for in a Social Worker:

  • Social Work England registration
  • Eligible to work in the UK
  • Hold a full UK licence
  • Postqualified experience
  • Degree level or equivalent in Social Work
