The Licensed Practical Nurse is responsible for utilizing the nursing process to provide for optimum quality and continuity of nursing care to patients in the comfort of their residence.

Responsibilities

Complete and scan required record keeping forms/charts upon the guests admission transfer and/or discharge.
Chart nurses notes in an informative and descriptive manner that reflects the care provided to the guest as well as the guests response to the care.
Report all discrepancies noted concerning physicians orders diet change charting error etc. to the Director of Nursing.
Ensure that appropriate documentation concerning unauthorized discharges is entered in the guests medical record in accordance with established procedures.

Qualifications

Current unencumbered LPN/LVN license required or Compact State license required.
A minimum of 2 years LPN/LVN experience required.
Able to educate patients and family on multiple disease processes and have good medication knowledge and wound care experience.
