Matchwell is hiring MDS Coordinators for one of our Skilled Nursing partners in Augusta GA. See below for more details.

Schedule: Fulltime Days
Position Requirements:
- BSN or ADN in Nursing from an accredited college or University.
- Current Active Georgia Licensure as a Registered Nurse in good standing.
- Current American Heart Association Healthcare Provider BLS/BCLS certification
- Five years of nursing experience with some experience with MDS programs and supervision
Essential Duties & Responsibilities:
- Develop care plans on all patients. Responsible for updating care plans Quarterly and Annual MDS assessments.
- Coordinate the development and completion of the patients assessment (MDS) in accordance with current regulations and guidelines. Participate in facility surveys made by VA State and the Joint Commission.
- Work with the interdisciplinary Care team to develop an accurate individualized comprehensive plan of care.
- Participate in planning inservice training classes that include assessment skills or techniques needed to complete assessment functions in LTC Component.
- Ensure that quarterly and annual patient assessments and care plan reviews are made on a timely basis.
- Ensure that all members of the interdisciplinary team are aware of the importance of completeness and accuracy in their assessment functions.
- Participate on committees as assigned by the ADON and DON.
- Perform all other related duties/tasks as assigned.
