Job Description:
Foundation Medical Partners is seeking a compassionate and dedicated Geriatrician to join our team and provide FullTime comprehensive medical care to our residents in a vibrant and supportive nursing home environment. As a Geriatrician with us you will play a vital role in ensuring the wellbeing and quality of life for our older adult population.
- Conduct comprehensive initial and ongoing evaluations of residents medical and psychosocial needs.
- Diagnose and treat a wide range of acute and chronic medical conditions common in older adults.
- Develop and implement individualized care plans in collaboration with nursing stafftherapistsand other healthcare professionals.
- Manage residents medicationsensuring safe and effective use.
- Provide family members and caregivers with education and support regarding their loved ones care.
- Participate in interdisciplinary care conferences and contribute to quality improvement initiatives.
- Stay up to date on the latest advances in geriatric medicine and evidencebased practices.
- Document medical records accurately and efficiently.
Qualifications:
- Boardcertified or boardeligible in Geriatric Medicine or Internal Medicine
- Valid medical license in the state of NH (or working towards)
- Minimum of 2 years of experience working in a geriatric setting (e.g.nursing homelongterm care facility)
- Excellent communicationcollaborationand interpersonal skills
- Compassionate and patient demeanor with older adults
- Strong commitment to residentcentered care and quality improvement
- Proficient with EPIC EMR
