Community Options Inc. is a national nonprofit agency providing services to individuals with disabilities in 12 states.
We are seeking aCommunity Integration Counselor in Carbon Monroe PA who will be responsible for assisting individuals with learning physical intellectual emotional and social life skills. The Community Integration Counselor provides individualized services designed to assist participants in managing and overcoming the difficulties confronted when living in the community.
Starting pay rate is: $16.50 an hour
Responsibilities:
- Support and encourage individuals to increase their ability to be as independent as possible
- Meet with waiver participants on a regular basis either in the individuals residence the community or an office setting
- Collaborate with the Case Manager to develop and facilitate programs which meet the social emotional physical and personal needs of the individual
- Provide ongoing support for community integration efforts
- Document individuals progress toward acquiring community integration
- Assist in building a support network between the individual and the community
- Ensure medical consultation forms are completed by the physician for each medical appointment attended
- Administer medications and ensures proper completion of necessary documentation
- Coordinates all leisure and volunteer opportunities
- Monitor the health and medical needs of individuals and immediately report any concerns
- Drive individuals to medical appointments and activities in the community
- Ensure program documentation and billable records are completed accurately and timely
- Must cooperate with licensee and department staff in any inspection inquiry or investigation
- Additional tasks and responsibilities may be assigned
Minimum Requirements:
- High School Diploma or GED
- Bachelors Degree preferred
- Valid drivers license with a satisfactory driving record
- Minimum 2 years of relevant experience supporting individuals with intellectual and developmental disabilities
- Ability to establish a comfortable and supportive relationship with individuals receiving supports
- Ability to exercise good judgment and remain calm in crisis situations
- Excellent verbal and written communication skills
- Excellent time management skills
Working Conditions:
- Transport individuals utilizing your own vehicle or company provided vehicles
- Schedule may change due to business needs and may include evening and weekend hours
- May be required to be oncall in cooperation with other management staff
- Frequent lifting stretching and other physical exertion may be required
- May be required to lift or move 25 pounds
- May be exposed to various medical conditions and communicable diseases
