Department:UHN Transitional Care Program 
Reports to:Manager UHN Transitional Care Program
Work Model: OnSite 
Salary:$26.95 33.69 per hour
Status: PFT/PPT/Casual/TFT/TPT
Closing Date: April 23 2025

Position Summary:

UHN Transitional Care Program is located at Queens Estate provides goal oriented shortterm care to clients from the community or acute care ALC or at risk of ALC who requires support to complete their restorative process and strengthen their functional capacity to return home or another discharge location.

Duties:

  • Provide education to patients and families regarding relevant deficits and related health promotion interventions
  • Works effectively independently as well as collaboratively with interprofessional team
  • Contributes to the care planning process and follows a designed plan of care
  • Provides safe competent care within Scope of Practice
  • Actively participates in education/mentorship of RPN students and new staff
  • Actively participates in professional program and hospitalwide initiatives
  • Compliance with and commitment to patient safety responsibilities and corresponding hospital plans policies and procedures in order to ensure a safe environment for patients

Qualifications :

  • Member in good standing with the College of Nurses of Ontario
  • Diploma preparation as an RPN in a recognized education program for Nursing preferred
  • Current medication certificate (if not included in diploma program)
  • Medical asepsis preferred (if not included in diploma program)
  • Experience in Complex Continuing Care preferred
  • Successful completion of Basic Cardiac Life Support (C level) Heart Saver Plus (Canada)
  • Member Registered Practical Nurses Association of Ontario (RPNAO) preferred
  • Well developed conceptual and theoretical understanding of nursing concepts as they relate to patient population and an ability to articulate these concepts
  • Well developed clinical decisionmaking skills that reflect patient centeredness problem solving and critical thinking
  • Excellent communication interpersonal and leadership skills
