Typical Qualifications

MINIMUM QUALIFICATIONS:

  • Bachelors degree in Human Services or a related field preferred.
  • At least one year of experience in community involvement and/or working with clients or an equivalent combination of education and experience.
  • Bilingual in English and Spanish required.
  • Valid Colorado Drivers License at time of appointment and throughout employment.
  • Knowledge of care coordination and community resources.
  • Proficiency in MS Excel MS Word MS Outlook and general database reporting systems.
  • Strong communication skills both oral and written with the ability to engage with clients and community partners effectively.
  • Ability to maintain confidentiality and handle sensitive information in accordance with Colorado State and Federal laws.
  • Ability to manage multiple tasks work independently and collaborate as part of a team.
  • Ability to perform physical tasks such as lifting bending walking and driving as required by the job.
EDUCATION EXPERIENCE AND FORMAL TRAINING:
  • Bachelors degree in Human Services or related field preferred and/or at least one year experience in community involvement and/or working with clients or an equivalent combination of education and experience.
  • Bilingual (English/Spanish)
  • Care Coordination
LICENSES OR CERTIFICATES:
  • Valid Colorado Drivers License at the time of appointment and throughout employment.
POSSIBLE WORK SCHEDULE:Flexible work schedule with regular hours being on weekdays and occasional weekends and evenings
