Job Title: Child and Adolescent inHome Therapist WellLIfe
Position Reports to Job Title: Site Supervisor
Program:Children
Location: Coram NY
Position Summary:
Provide clinical services including individual family and group therapy to children and families.
Essential Accountabilities:
- Provide inperson offsite clinical services including individual family and/or group therapy to children and their families.
- Conduct evaluations diagnose health impairments and identify needs of children and adolescents.
- Provide crisis services inperson or via phone to caseload when available.
- Complete progress notes for each session and telephone calls.
- Complete and maintain treatment plans for children and youth on caseload.
- Coordinate with childs Care Coordinators or other providers as needed.
- Deliver and document services using a strengthbased and familydriven approach.
- Assume other responsibilities assigned to support the operations of the program
Qualifications:
The preferred candidate will be a Licensed Practitioner of the Healing Arts including Licensed Psychoanalysts Licensed Clinical Social Workers Licensed Marriage and Family Therapists and/or Licensed Mental Health Counselors.
Clean and valid NYS drivers license and access to a vehicle.
