Your Role: Director of Case Management As the Director of Case Management you will be at the forefront of driving excellence in patient care and optimizing healthcare delivery. Your responsibilities will include:
Leadership: Manage and lead the Case Management Department overseeing various functions including Bed Board/Clinical Intake Disease Management Social Services Discharge Planning Precertification and Denial Management. Inspire guide and mentor a talented team of professionals.
Strategic Planning: Set strategic goals performance standards and operating policies to ensure the highest quality of patient care. Interpret and implement hospital policies regulations and standards effectively.
Innovation: Drive innovation in case management discharge planning and care coordination. Identify opportunities for cost reduction without compromising patient care quality.
Collaboration: Coordinate negotiate and procure services and resources to optimize the care of complex patients. Foster collaboration among interdisciplinary teams to achieve efficient and effective patient outcomes.
Evaluation: Assess the effectiveness of Case Management services especially related to reimbursement for inpatient and outpatient care. Use datadriven insights to continuously improve processes.
Development: Develop clinicallybased case management strategies that enhance the delivery of quality healthcare. Ensure appropriate resource utilization across the continuum of care.
Qualifications:
- Bachelors or Masters degree in Healthcare Administration or related field.
- Extensive experience in healthcare management and case management leadership.
- Strong understanding of healthcare regulations policies and reimbursement processes.
- Exceptional communication leadership and teambuilding skills.
- Visionary mindset with a commitment to driving positive change.
