Location
Job Description
1. Develops and effectively utilizes a network of information regarding community resources.
2. Coordinates the discharge planning process in collaboration with other professional members of the interdisciplinary team.
3. Appropriately delegates within the scope of practice discharge planning activities and functions to others involved with discharge planning; Case Management associates or other nonlicensed personnel performing discharge planning activities.
4. Identifies the appropriate level of posthospitalization care and services required and makes referrals to alternate levels of care as needed.
5. Communicates and documents discharge planning needs in the medical record.
6. Provides necessary patient teaching relevant to discharge needs and/or posthospital care arrangements prior to discharge.
7. Collaborates with the family and medical team to make appropriate referrals to external vendors for concrete discharge planning needs including but not limited to nursing durable medical equipment and alternate care facilities.
8. Keeps the interdisciplinary care team informed regarding details of the discharge plan.
9. Communicates essential information to the next care provider.
10. Coordinates internal and external services to avoid under or over utilization of resources.
11. Refers cases to the Case Management Medical Director as needed and documents the referral.
12. Identifies avoidable delays in care or delays in discharge attempts to resolve delays and document identified avoidable days in MIDAS on a daily basis
13. Understands the target LOS on every patient at the time of the initial review and identifies the anticipated date of discharge for planning and care coordination purposes.
14. Maintains an open line of communication with other disciplines regarding the target LOS and anticipated discharge date.
15. Documents all team physician and patient/family communication and concerns pertaining to coordination of care and services in the medical record.
16. Participates in unit bed huddles and multidisciplinary rounds.
17. Assists medical team and family in coordinating post hospitalization followup appointments.
18. Demonstrates responsibility for professional development and growth including identifying continuing education needs and seeking learning opportunities that will enhance job performance and professional satisfaction.
19. Effectively organizes work priorities.
20. Demonstrates compliance with facility and departmental safety and security policies and practices.
21. Educates hospital team members and physicians about the Clinical Case Manager role and scope of RN practice.
22. Other job functions as assigned.
Minimum Qualifications:
1. Graduation from a BSN program.
2. License to practice as a RN in Illinois
3. Minimum of 2 years of RN or Case Management practice.
4. Certification as a Case Manager preferred.

Pay Range
$83200.00$137280.00 SalaryAt Lurie Childrens we are committed to competitive and fair compensation aligned with market rates and internal equity reflecting individual contributions experience and expertise. The pay range for this job indicates minimum and maximum targets for the position.
