Starting Salary

The starting pay is $18.27 per hour. To qualify for a higher pay rate the candidate will have to provide documentation of relevant experience and qualifications.

Job Overview

As a Community Navigator youll play a pivotal role in empowering individuals with developmental and/or physical disabilities. This position involves working closely with participants their support teams and service providers to develop PersonCentered goals and objectives. Youll facilitate meaningful community engagements ensuring each participants safety and personal growth.

Key Responsibilities

  • Conduct assessments to determine participants interests and needs.
  • Coordinate with a diverse team to support participants integration into the community.
  • Advocate for participants ensuring they have access to necessary services and support.
  • Develop implement and monitor communitybased programs that align with participants PersonCentered Plans.
  • Serve as a liaison between participants their families and community resources.
  • Lead and support staff in delivering highquality personcentered services.

Qualifications

Qualifications

  • Deep understanding of learning methods and services for adults with disabilities.
  • English and Spanish bilingual preferred.
  • Strong case management skills and experience in community program development.
  • Exceptional leadership team building and interpersonal skills.
  • Proficiency in personal computer use and relevant software applications.
  • Ability to work independently make wellinformed decisions and manage multiple projects.
  • Bachelors degree or equivalent in a related field preferred.

Working Environment

  • Open comfortable office setting with occasional community travel.
  • Dynamic and supportive team atmosphere.

Work Schedule

FullTime Position Monday through Friday
