- 3 years of experience in health care management or home care
- Demonstrated ability to provide professional guidance based on your knowledge and experience in the field of aging and care management best practices
- Knowledge of all aspects of the aging process and conditions affecting older adults and adults with disabilities including memory care
- Impeccable attention to detail and strong follow through
- Integrity honesty and commitment to high standards of excellence and professional ethics
- Commitment to working collaboratively with a multidisciplinary team
- Excellent listening skills and communication skills (written and verbal)
- Ability to prioritize work and manage time effectively
- Proficiency in Microsoft Office Suite Google Drive Google Chrome
- Available weekdays
- Valid California drivers license vehicle insurance personal transportation (care managers work from home and travel to clients homes)
