Overview:
The Advanced Practice Provider (APP) either a Nurse Practitioneror Physician Assistant plays a critical role in delivering comprehensive care to participants in the PACE program. Collaborating with physicians and the Interdisciplinary Team (IDT) the APP provides participantcentered care across various settings including the PACE Day center homes hospitals nursing homes and assisted living facilities. Focused on preserving health independence and quality of life the APP conducts assessments diagnoses treats and coordinates care for an aging population. The role requires adherence to professional licensing state regulations organizational policies and PACE program standards.
Primary role responsibilities of the NP/PA include:
- Coordinate patient care with the medical team interdisciplinary team (IDT) and external agencies.
- Participate in daily IDT meetings to develop and review care plans.
- Perform routine health assessments to monitor participant wellbeing.
- Assess and treat acute health concerns; manage chronic disease states across various care settings.
- Lead Advanced Care Planning discussions with participants and caregivers.
- Educate participants and families on chronic conditions to promote care engagement.
- Collaborate with community partners to ensure seamless care transitions.
- Participate in quality improvement initiatives and provide oncall support as needed.
- Uphold the organizations values and contribute to a positive workplace culture.
- Communicate effectively with individuals of all ages especially the geriatric population.
- Thrive in a selfdirected flexible work environment.
- Demonstrate strong organizational and problemsolving skills.
- Work collaboratively within a multidisciplinary team.
- Proficient in using electronic medical records (EMR).
What it takes:
- Current license and certification to practice as a Nurse Practitioner (NP) or Physician Assistant (PA) in the State of Michigan
- Masters degree in nursing or satisfactory completion of a physician assistant program
- BLS certification
- Current DEA registration
- State of Michigan Controlled Substance License
- Minimum of 1year experience with the frail and elderly or longterm care population preferred
