The Department of Family Medicine and Community Health is currently seeking a full time Case Worker to work in our Miami campus. Case Worker coordinates services for respective population including providing information about professional development opportunities quality improvement strategies and counseling to improve clients social and psychological conditions. Moreover the Case Worker provides assistance to those experiencing difficult situations by helping them find the community resources/services that they need.

Any appropriate combination of relevant education experience and/or certification may be considered.

Department Specific Functions

General Description (Purpose and Function):

The Case Worker (CW) will assist patients with their tobacco cessation efforts and will educate health care providers on tobacco cessation best practices and the use of appropriate pharmacotherapy products. The CW will plan conduct and evaluate support groups for patients who are trying to quit smoking at various clinical settings and in coordination with their health care providers. The CW will provide tobacco prevention education and information and support community efforts to create/sustain tobacco free environments and worksite wellness programs for employees in partnerships with the organizations.
