Job Summary
Job Summary: The Department of Pediatrics in the University of Utah School of Medicine has an immediate opening for a Clinical Nurse Coordinator. The Nurse Coordinator works in conjunction with other team members in a teambased model of care assessing planning coordinating and evaluating care services to ensure safe and effective patient care. Additional activities include assisting with staff development/education specific to children with medical complexity quality improvement activities and performing other duties to facilitate care for patients. The Nurse Coordinator will work with highly skilled team members to provide high value patient care in an organization focused on patient safety patient and parent satisfaction and positive patient outcomes. Responsibilities
Responsibilities Coordinate with families and providers regarding the care of children with spasticity and medical complexity including those receiving intrathecal baclofen therapy and neurolytic injections. Advocate for patients needs by coordinating with insurance companies and outside community service agencies. Assess patient and family needs for clinical services; communicate directly and indirectly with healthcare providers in order to appropriately coordinate all aspects of patient care. Document in the EMR appropriately to meet the requirements of Chronic Care Management CCM . Provide continuous care management with patients families and other stakeholders including oversight of care plans coordination of care communication of lab and other diagnostic studies as well as triage and highly accessible support for acute and/or urgent concerns. Collaborate closely with team members internal and external stakeholders and support services to provide high value care to children with disabilities and their families. Coordinate outside records with the EMR and manage clinical information for process improvement and quality initiatives. Coordinate medication refills letters of medical necessity DMV forms school care plans and medication prior authorizations as needed. Maintains inventory of specialized medications or items and ensures items are available. Utilize evidence/researchbased nursing guidelines to ensure superior nursing care. Perform other related duties as assigned. Work Environment and Level of Frequency that may be required: Nearly Continuously: Office environment. Often: Infectious disease oils there is air or skin exposure to oils or other cutting fluids hazards includes a variety of physical conditions such as proximity to moving mechanical parts moving vehicles electrical current working on scaffolding and high places or exposure to chemicals . Physical Requirements and Level of Frequency that may be required Nearly Continuously: Hearing listening talking. Often: Repetitive hand motion (such as typing) walking standing. Minimum Qualifications
Required Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system. The eCredential is to be obtained within 30 days of hire. Current license to practice as a Registered Nurse in the State of Utah or obtain one within 90 days of hire under the interstate compact if switching residency to State of Utah. Must maintain current Interstate Compact (multistate) license if residency is not being changed to Utah. Two years general nursing experience. Preferred Bachelors degree in Nursing or equivalency (one year of education can be substituted for two years of related work experience). One year of directly related experience in an area of specialty is preferred. Some positions may require additional certifications such as Advanced Cardiac Life Support ACLS Pediatric Advanced Life Support PALS or Neonatal Resuscitation Program NRP within one year of hire.
