Job Description
Job Title: Adults Services Social Worker Locality Team
Location: Gateshead
Rate: 154.99 per day
An exciting job role has recently become available for a dedicated and passionate Adults Services Social Worker to join a Locality Team in Gateshead. The successful candidate will play a crucial role in overseeing care management across the borough providing essential services to individuals with physical and/or organic mental health needs.
Your Responsibilities:
You will conduct complex assessments of individuals needs and circumstances. You will develop and implement comprehensive care plans and deliver targeted interventions to support individuals in managing their needs. Additionally you will work collaboratively within a team that covers half of the Borough and undertake assessments facetoface in a hybrid working environment.

Qualifications and Experiences:
To be eligible for this role you must have Social Work England registration and be eligible to work in the UK. Additionally you need to hold a full UK driving licence and possess postqualified experience. A degree or equivalent qualification in Social Work is also required.
