Job Description

Title: Bilingual Hospice Social Worker

Position Type: Permanent Fulltime
Location: Bronx and Queens
Hours: Monday Friday 9:00am 5:00pm
Salary: $68838K82605K plus $15000 Signon Bonus or Student Loans Assistance paid over the course of 3 years

Our client seeks to be the premier provider of health services in the greater metropolitan area ensuring access to health supportive and communitybased services across the continuum of need. Their range of health services include home care hospice and palliative care for adults and children rehabilitation and nursing care and the researchbased institution for innovation in palliative care. They also offer health plans for Medicare and dualeligible individuals. Their ability to serve atrisk members of the community is supported by the efforts of their nonforprofit foundation which raises funds to help support their quality programs and services not reimbursed by insurance or government programs. In addition the generosity of grateful families private and corporate donors and grants as well as their own employees also support their specialized services.

Tasks

Responsibilities:
Our clients groundbreaking hospice and palliative care programs offer a significant difference when dealing with a lifelimiting condition. They offer a broad range of services in the community or facilitybased to help patients and their families ease the burden of managing a lifelimiting illness or end of life care.

  • You will provide psychosocial support and other Social Work services to Hospice patients and their families in the inpatient home care/nursing home settings in pre and post bereavement phases of Hospice service
  • You will serve as social work member of hospice IDG and as liaison between hospice staff and inpatient and nursing home staff
  • You will develop social work plan of care goals and makes recommendations based on patient/family needs for assigned patients who are direct admission to inpatient units or nursing home/hospice residents.
  • You will develop social work plan of care and discharge plans when appropriate for patients admitted into inpatient facility.

Requirements

Requirements:

  • Graduate from an accredited School of social work.
  • Bilingual skills required (Cantonese or Mandarin)
  • Minimum of one year experience in Social Work in a healthcare setting.
  • Knowledge of Word and excel preferred.
  • New York State MSW licensure.
  • Valid NYS Drivers license and car required.
  • Ability to work within an interdisciplinary team environment.
  • Ability to input clinical data proficiently.
