Job Title:Social Worker Experienced Hospital Discharge
Location:London
Rate:Up to 35 per hour
An exciting job role has recently become available at the North London Local Authority. They are looking for a new member for their Hospital Discharge Team.
What will your responsibilities be
The team is seeking an enthusiastic individual to engage in strengthsbased personcentered assessments. This role involves managing a varied caseload including capacity assessments best interest decisionmaking Care Act assessments and participating in a duty rota. The successful candidate will deliver exceptional social care to adults and their carers promoting choice and control as much as possible. Additionally they will engage in outcomesfocused and preventative conversations that build on the strengths of individuals and their local networks.

Qualifications and Experiences:
You must have a degree in Social Work and be registered with the SSSC. Additionally you should possess knowledge of relevant legislation and policies. Excellent communication and interpersonal skills are also essential for this role.
