Position Overview:

We are seeking a dedicated MSW (Master of Social Work) to join our team and provide Community Comprehensive InHome Care (CIC) and/or Independent Living Skills Training (ILST) services. The MSW will work with individuals directly to develop and implement personcentered plans that support the individuals goals and preferences and promote independence and community integration. and nbsp;

Responsibilities: and nbsp;
and nbsp;
Conduct initial and ongoing assessments to identify the individual needs preferences strengths and challenges. and nbsp;
Develop and implement personcentered plans that address the individual goals and preferences and promote independence and community integration. and nbsp;
Provide ongoing support and coaching to individuals in their homes or community settings as specified in the personcentered plan. and nbsp;
Assist individuals with daily living tasks such as meal preparation medication management and personal hygiene. and nbsp;
Provide training to individuals and their families on topics such as budgeting household management and communication. and nbsp;
Coordinate with other service providers involved in the individuals care such as healthcare professionals therapists and case managers. and nbsp;
Implement and monitor Positive Behavioral Interventions and Supports (PBIS) strategies in collaboration with the individual and their team. and nbsp;
Provide Independent and nbsp;Living and nbsp;Skills and nbsp;Training and nbsp;(ILST) and nbsp;to and nbsp;individuals and nbsp;to and nbsp;promote their independence and selfsufficiency. and nbsp;
Maintain accurate and timely documentation of services provided progress towards goals and other relevant information. and nbsp;
Attend team meetings and other required trainings and meetings. and nbsp;
Maintain confidentiality and adhere to ethical and professional standards. and nbsp;
and nbsp;
Qualifications: and nbsp;
and nbsp;
Bachelors degree in social work psychology or a related field. and nbsp;
Experience working with individuals with traumatic brain injuries and nbsp;
and/or disabilities. and nbsp;
Knowledge of the NHTD program and regulations. and nbsp;
Strong communication and interpersonal skills. and nbsp;
Ability to work independently and as part of a team. and nbsp;
Proficiency in computer skills including electronic health record and nbsp;
systems. Valid drivers license and reliable transportation. and nbsp;
