We are seeking a dynamic and experienced Director of Case Management to join our team and lead our efforts in delivering highquality patientcentered care. Key Responsibilities:
As the Director of Case Management you will play a pivotal role in overseeing and optimizing our case management programs. You will ensure the delivery of efficient effective and compassionate care while driving continuous improvement in patient outcomes. Your responsibilities will include:

  • Leading and mentoring a team of case management professionals.
  • Developing and implementing strategies to enhance care coordination and resource utilization.
  • Collaborating with interdisciplinary teams to ensure seamless patient care transitions.
  • Monitoring compliance with regulatory standards and organizational policies.
  • Analyzing data to identify trends and opportunities for improvement.
  • Fostering a culture of excellence innovation and accountability.

Qualifications & Requirements:

  • Associates Degree in Nursing (required); Bachelors or Masters Degree in Nursing or related field (preferred).
  • Current RN license to practice in North Carolina.
  • Proven ability to think critically make decisive judgments and work independently with minimal supervision.
  • Strong leadership skills with a minimum of 3 years of management experience (preferred).
  • Ability to thrive in a fastpaced highpressure environment and take appropriate action when needed.
  • Excellent communication interpersonal and problemsolving skills.
