Job Description
Job Title: Qualified Adults Social Worker Hospital Discharge Team
Location: East London
Rate: Up to 35.00 per hour
An exciting job role has recently become available in East London. They are looking for a new member for their Hospital Discharge Team.
What will your responsibilities be
You will manage a varied caseload including capacity assessments best interest decisionmaking Care Act assessments and being part of a duty rota. You will deliver exceptional social care to the adults you are supporting and their carers promoting choice and control as much as possible. You will engage in outcomesfocused and preventative conversations that build on the strengths of people and their local networks.

Qualifications and Experiences:
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK licence
- Postqualified experience
- Degree level or equivalent in Social Work
