Long Term Care Pharmacist

We are actively seeking a qualified and dedicated Pharmacist to join our remote team with a focus on LongTerm Care (LTC). In this role you will play a vital part in optimizing medication therapy for longterm care residents collaborating with healthcare teams and ensuring adherence to regulatory standardsall from the convenience of your remote work environment. The ideal candidate will possess expertise in longterm care pharmacy practice excellent communication skills for remote collaboration and a commitment to providing exceptional pharmaceutical care.

Preferred Qualifications:

• PharmD degree from an accredited pharmacy program
• LongTerm Care pharmacy experience
• State pharmacy license in good standing (remote work eligibility)
• Clinical pharmacy expertise with a focus on geriatrics
• Strong analytical and problemsolving skills
• Excellent communication and collaboration abilities for remote interactions
• Familiarity with electronic health record (EHR) systems used in LTC settings
• Dedication to ongoing professional development and continuous improvement

Responsibilities:

• Provide remote clinical pharmacy services tailored to the unique needs of longterm care residents
• Collaborate remotely with healthcare teams including physicians and nursing staff
